Why do most guides say nine components?

Because the chain grew. Pre-1.4.4, the Ankh Charm folded nine debuff trinkets; 1.4.4 added the Reflective Shades — Blindfold + Pocket Mirror (Medusa drop) — which brought Stoned immunity into the recipe and took the base count to twelve (10 drops + Obsidian + Cobalt Shield). If a guide tells you the Ankh Shield needs nine items, it predates 1.4.4 — the current tree on any 1.4.5 world needs the Medusa farm.

What does the finished shield do?

The completed shield stacks eleven debuff immunities — Weakness, Broken Armor, Bleeding, Poisoned, Silenced, Cursed, Confused, Slow, Chilled, Stoned, and Blindness — on top of the Obsidian Shield’s knockback immunity and +defense. It is the strongest single defensive accessory in the game and the anchor of most endgame accessory loadouts; pair it with the Zenith and a Solar Flare set and the character is functionally armored against every common debuff.

How should I farm it?

The efficient order is by biome, not by item: Hardmode Dungeon for Armor Polish + Adhesive Bandage + Nazar-adjacent cursed weapons and the Cobalt Shield itself; a Desert or Corrupt/Crimson surface farm for the mummy drops; the Jungle for Bezoar; the Hallow for Pixie pieces; and a Medusa hunt for the Pocket Mirror. A water-candle battle-potion grind in each zone beats running them one at a time. The pieces drop pre-Plantera — the shield is earnable well before the endgame boss line on the boss order, and finishing it before Plantera makes her poison and Thorn Ball phases far more forgiving.
